According to the investigation, the crash transpired when a 2015 Nissan Altima, driven by 42-year-old Sulmu Larissa Zelaya Moreno from Fort Worth, MO, failed to stop at a stop sign. This failure to yield resulted in the Altima colliding with a 2020 Ford Escape, which was being driven by 85-year-old Mary K. Brooks from Pittsburg, MO. The impact was severe enough to total both vehicles, which were subsequently towed by Stauffers Towing.

Sulmu Larissa Zelaya Moreno and her passenger, 38-year-old Deris Guevara Velasquez, also from Fort Worth, TX, both sustained serious injuries and were transported by ambulance to Lake Regional Hospital in Osage Beach, MO. Mary K. Brooks and her passenger, 90-year-old Clifton E. Brooks, were also injured, with both being transported to Cox South Hospital in Springfield, MO.
