The incident, initially reported as a two-vehicle head-on collision, occurred around 5:23 PM and escalated as responders arrived on the scene.

Emergency responders from Stations 15 and 45, including RQ-15 and EN-15, quickly took action. Reports indicated no entrapments, and medics assessed and treated individuals for injuries. At least one individual was transported to Butler Memorial Hospital (BMH) for further care, while another declined transportation after being evaluated at the scene. Traffic was heavily impacted as the accident blocked all lanes, prompting Station 15 to implement one-lane traffic control measures at the top of a nearby hill.
