Putnam County, OH (January 28, 2025) – A tragic two-vehicle collision in Putnam County claimed the life of 32-year-old Erica Shafer of Continental, Ohio, and left three others seriously injured on Monday evening.

The Ohio State Highway Patrol (OSHP) reported that the crash occurred around 8:25 p.m. when Shafer, driving southbound on County Road 21, failed to stop at a stop sign at the intersection with State Route 15. Her vehicle collided with an eastbound Chevrolet Suburban on SR 15.

The impact caused both vehicles to leave the roadway, striking a utility pole, a traffic sign, and a junction box. Shafer, who was not wearing a seatbelt, was pronounced dead at the scene.

The driver of the Chevrolet Suburban and two juvenile passengers, who were all wearing seatbelts, sustained serious injuries and were transported to a nearby hospital via ambulance. Their current conditions have not been disclosed.

Authorities confirmed that alcohol impairment was a factor in the crash. The intersection was closed for two hours as crews worked to investigate and clear the scene.

Our deepest condolences go out to the family and loved ones of Erica Shafer. We also send our thoughts to the injured individuals and wish them a swift recovery.

Intersection Crashes in Ohio

Intersections are a common site of severe collisions, particularly when drivers fail to obey traffic controls such as stop signs. Factors like alcohol impairment, speeding, and failure to yield significantly increase the risk of accidents at these locations, as demonstrated in this tragic crash at County Road 21 and State Route 15.

Wearing seatbelts is critical to reducing the severity of injuries in motor vehicle accidents. This incident underscores the importance of both adhering to traffic laws and prioritizing safety measures, such as proper seatbelt use.
